The short version
- The early access form asks for your name, email, and a few questions about how you work. That is all we collect through it.
- We use it to contact you about your request and to understand what independent clinicians need.
- We never collect license numbers, Social Security numbers, DEA numbers, or documents through this website.
- You can ask us to delete your information at any time.
What we collect
When you fill out the early access form: your first and last name, email address, whether you are a CRNA, a physician, or something else, the state where you mainly practice, how many facilities you work at, how many new facilities you start at in a typical year, which parts of credentialing are a headache for you, anything you choose to write in the notes box, and your agreement to be contacted.
Along with the form: the page you were on and, if the link you followed included them, referral and campaign tags (for example, a ref or utm_source value in the link). These tell us how you found us.
On the credentialing checklist page: if you tick boxes, your browser remembers which ones so they are still checked next time. That stays on your device. It is never sent to us.
When you browse the site: if analytics is turned on, Google Analytics collects standard usage information, such as the pages you visit, your general location, your device and browser type, and how you arrived. Google Analytics uses cookies to do this.
What we do not collect
Please do not enter license numbers, Social Security numbers, DEA numbers, or any documents through this website. We will never ask for them through the form or by email. If you become a client, we will set up a secure process with you before collecting any credentials or documents, and that work will be covered by a separate agreement.
How we use it
- To reply to your request and set up a short call.
- To understand which credentialing tasks matter most to independent CRNAs and physicians, so we can shape the service.
- To understand how visitors find and use the site.
We do not sell the information you submit, and we do not use it for advertising.
Who sees it
ClearGate is run by one person, Matthew Pierce, who is the only person at ClearGate who sees form submissions.
Your information is also processed by the companies that run this website: our website host and form processor (Netlify) and, if analytics is turned on, Google. We may also use other tools to organize follow-up, such as an email provider or a private database. These companies process data under their own terms and privacy policies.
How long we keep it
We keep early access requests for up to 24 months after our last contact with you, then delete them. If you become a client, the agreement for that work will say how your information is handled from then on.
Your choices
